Position Open: County Agency Social Worker
Southwest Health and Human Services currently has an opening for a County Agency Social Worker. This position can be in any of our locations.

Long Term Care Services and Supports for the Elderly including assessments, Elderly Waiver and Alternative Care case management services and It also includes care coordination for Minnesota Senior Health Options and Minnesota Senior Care Plus program for community well, elderly waiver, and nursing home residents.
Minimum Qualifications:
• A bachelor’s degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.
or
• A bachelor’s degree from an accredited four year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency.

Starting Rate of Pay: $24.36/hour
