Southwest Health and Human Services currently has an opening for an Adult Mental Health (AMH) Social Worker. This position is located in our Redwood Falls location.

The primary purpose of this position is to provide adult mental health targeted case management to eligible consumers. The position is responsible for statutory requirements of Rule 79 Case Management. The position will meet with clients who qualify for Rule 79 Case Management at least once a month, sometimes more, depending on the consumer’s needs. During these meetings, the case manager will determine the client needs by completing Functional Assessments, reviewing Diagnostic Assessments and communication with consumers. The position is responsible for completing Individual Community Support Plans (ISCP) with the consumer and connecting consumers and supportive community resources tailored to their needs. This position will back up the commitment position and will also conduct OBRA II screenings for person with mental Health diagnosis entering a NH setting.
Minimum Qualifications:
- A bachelor’s degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field.
or
- A bachelor’s degree from an accredited four year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency.

Starting Rate of Pay: $25.60/hour in 2024 and $26.10/hour if starting in 2025
